Taking this questionnaire and submitting it to Traci prior to the initial appointment would be helpful. If that is not an option, taking this questionnaire and having it ready for your initial meeting with Traci is acceptable as well. This will help Traci assess your health, lifestyle and things that you want to work on together. All information is classified, and will not be shared with any outside parties, unless otherwise agreed upon.
